Please fill out the appropriate Game Plan form below. This form should be completed a few days prior to your planning meeting with your counselor!

Student Questionnaire for Counselor Recommendation
 
Brag sheets are a way for you to help your counselor and teachers write letters of recommendation. The form give them more insight into who you are outside of school and your interests. Fill out the form and share them with your counselor when you ask them for a letter of recommendation!

Please fill this out to give your counselor more information moving forward about your student. This will help counselors in writing letters of recommendation and more.
