Registration and Physical Information for
 2020 Fall sports season
Physical forms and Re Eval (HHQ) forms can be found on the link at the bottom of this page.  A physical is due every 365 days in order to participate in athletics.  This year, due to COVID-19,  fall athletes that have a completed and cleared physical on file in the nurses office from last year will be able to be cleared to start athletics by filling out the 2020 Updated Re eval form found at the bottom of this page.
This will allow a fall athlete to begin practice but they will still need a new physical as soon as they can get one
cleared with the school district before the fall season ends.
The following must be handed in before any athlete can start practice on July 13  (all forms can be found at the bottom of this page)
The forms also have to be cleared by the school nurse so please hand them in as soon as possible.

  • 2020 Updated Re-eval ( Health History Questionnaire)   or PPE Form 
  • The COVID-19 Questionnaire must be handed in with your PPE form or Re-eval once
  • Covid Daily Questionnaire-    This form must be handed in each day before practice.

All Athletes must  sign up to participate in sports through the Parent and Student Portal once your physical/re eval is cleared (Parent Portal will not be open until August)



Information below when filling out the PPE form

Prior to participation on a school-sponsored interscholastic or intramural athletic team or squad, each student-athlete in grades six through 12 must present a completed pre-participation physical evaluation (PPE) form (attached)  to the School Nurse.  Important information regarding the PPE is provided below, and you should feel free to share with your child’s medical home health care provider.

MANDATORY!!   The PPE may ONLY be completed by a licensed physician, advanced practice nurse (APN) or physician assistant  (PA)- that has completed the Student-Athlete Cardiac Assessment professional development module.  It is recommended that you verify that your medical provider has completed this module before scheduling an appointment for a Physical.


ACADEMIC REQUIREMENTS need to be met for high school athletes to be eligible to participate. The NJSIAA requires that all high school students earn 30 credits in the previous school year to be eligible for fall and winter interscholasatic sports and 15 credits in the first semester of the current school year to be eligible for spring sports. Senior not carrying enough classes to earn 15 credits must be passing all their classes and be on track for graduation.



Travel Release Forms

Students must have permission to drive to the Field House (or be transported to the Field House) by the parent.  The travel Release forms are located below.
The school provides busing to all games and contests and in order for an athlete to be driven by their parent they must fill out the form below (Travel Release Form- GAMES) 2 days prior to the event and hand in to the athletic office.
